Non-JOBZ Business Assistance Form
The Non-JOBZ Business Assistance Form should be used under the following conditions:

• If non-JOBZ project activity for business subsidies exceeds $150,000 for business assistance agreements signed between January 1, 2008 and December 31, 2011
• If the government agency represents a population of more than 2,500
• If they have awarded business subsidies during the last five years
• If business subsidies valued at $25,000 to $150,000 and/or business assistance loans valued at $75,000 to $150,000 for agreements signed between August 1, 1999 and December 31, 2007
