CARF-Accredited Community Rehabilitation Providers
VR counselors can authorize services from these providers with no volume limits. When a Community Rehabilitation Program (CRP) achieves accreditation, it means the provider has passed an in-depth review of its business practices and accredited service areas. Accreditation is an assurance that the provider meets rigorous CARF guidelines for service and quality -- a qualified endorsement that the provider conforms to nationally and internationally recognized service standards and is focused on delivering the most favorable results for persons served. Tips for CRPs
Retain preferred vendor status; prevent lapses in Operating Agreements and referral delays; and always get the help you need, when you need it by noting these items:
CARF Survey Application
Submit the Intent to Survey for your next survey three months before the two month time frame requested for survey, this is usually 5-6 months before your current accreditation expires. Include all employment services and service sites that you want included on your organization’s new Operating Agreement. Copy your Field Representative.
Survey Preparation and Assistance
Rehabilitation Services employs the following staff members, who are also CARF Surveyors, and who may be available, on a first-come-first-serve basis, to provide technical assistance as your organization prepares for its upcoming CARF survey. There is no charge to CRPs, but availability may be limited. For assistance contact:

CARF Survey Report
Your current Operating Agreement will automatically expire on the date your current CARF accreditation expires. To ensure VR Counselors have the ability to make referrals to your organization, forward a copy of the new CARF Survey Report as soon as possible to your Field Representative, who will assist you in establishing a new Operating Agreement for the services and locations identified on the CARF Survey Report.
